If you are interested in having your student attend SCVi, please download and fill out an application below, or pick one up from the school at:

The complete application must be turned in in person at the school between 9:00 and 3:00.

If you are interested in joining the waiting list for the current school year, please follow the same instructions, but specify to the office staff that this if for the current year.

APPLICATION DOCUMENTS

2010/2011 school year forms are due March 19, 2010 to be included in lottery held March 26, 2010.

Families turning in paperwork after March 19 will go to the end of the waiting list after the lottery. Turning in paperwork before March 19 does not increase your chances of being enrolled in the random lottery.

If you are on this years waiting list, you must re-enter the lottery and fill out a new form.

It is highly recommended that you tour the school and/or attend a parent informational night before choosing to send your student to SCVi. You are not required to do this before you enter your family's name in the lottery.

Parents must attend a mandatory orientation for their child to attend SCVi. See the events calendar for dates. Usually this is the same nigh as Back To School Night.

Special Needs Students
If your student has an Individualized Education Plan, please fill out a 09/10 registration form and turn it in.

After the random lottery, if your student is offered a spot, we will hold a transition IEP with the Hart District (they provide Special Ed Services for SCVi), the District the student is coming from, the parents and SCVi staff to assess as a team, if we can meet your students needs at our site.

Your spot will not be given away during this process. Please note: we do not have Special Day Classes.

Returning Students
Because open enrollment to the public has begun - with a lottery being scheduled for March 26, 2010 - we would like to plan early for the upcoming school year and would like to reserve a space for your child for the 2010-2011 academic year. Please take a moment to complete the attached re-enrollment form and return it to SCVi with your child or hand deliver it to the office no later than February 19, 2010. No further actions on your part will be required for re-enrollment.
